OVERVIEW
Our Admission Process
AFLH utilizes a structured referral and admission process to ensure clinical appropriateness, safety, and program fit.
All referrals undergo review by qualified personnel prior to acceptance. Our goal is to ensure that each individual receives services aligned with their level of care needs.
We coordinate closely with case managers, hospitals, families, and referral sources to support smooth transitions into care.
WHO MAY SUBMIT A REFERRAL
Referrals may be submitted by:
• County agencies
• Case managers
• Hospitals and discharge planners
• Behavioral health providers
• Guardians or family members (when appropriate)
All referrals are reviewed for eligibility and program alignment.
LEVELS OF CARE AVAILABLE
AFLH provides services across multiple levels of care:
• Adolescent Residential Treatment
• Adult Residential Support
• Outpatient Behavioral Health Services
• Day Treatment Programming
• Case Management & Services Coordination
During review, we determine the most appropriate service level based on clinical information provided.
REFERRAL REVIEW PROCESS
Step 1 – Referral Submission
Complete the secure online referral form with supporting documentation.
Step 2 – Clinical & Program Review
Our team evaluates appropriateness, level of care needs, and safety considerations.
Step 3 – Additional Information (if needed)
We may request supplemental records, assessments, or discharge summaries.
Step 4 – Admission Decision
Referral sources are notified of acceptance, denial, or alternative recommendations.
REQUIRED DOCUMENTATION
Documentation may include:
• Recent behavioral health assessment
• Medication list
• Discharge summary (if applicable)
• Insurance or funding information
• Legal custody documentation (for adolescents)
Incomplete referrals may delay review.
ADMISSION COORDINATION
Once accepted, our team coordinates:
• Admission scheduling
• Transition planning
• Family or guardian communication
• Transportation coordination (if applicable)
• Initial assessment scheduling
We prioritize safe and organized transitions into care.
EMERGENCY OR CRISIS SITUATIONS
AFLH does not function as an emergency facility.
If an individual is experiencing an immediate crisis, please contact 911 or the appropriate crisis intervention service prior to referral submission.

AFLH uses different submission processes based on level of care.

Option 1: Youth Referral Submission
Adolescent residential treatment placements are referral based. Referrals are typically submitted by county agencies, hospitals, case managers, or guardians with supporting documentation.
All youth referrals undergo clinical review prior to admission determination.
Option 2: Adult Residential Support
Adult residential services begin with a structured intake process to determine eligibility and residential appropriateness.
Intake submissions may be completed by the individual, family member, or authorized representative.
Following intake review, our team will contact the appropriate party to coordinate next steps.
