ADMISSIONS AND REFERRALS
PLACEMENT REVIEW AND SERVICE ELIGIBILITY
Referral Overview
AFLH accepts referrals for residential support services, residential treatment, outpatient treatment, day treatment, and case management services.
All referrals are reviewed to determine:

Service eligibility

Appropriate level of care

Regulatory scope alignment
-
Availability of program capacity
Submission of a referral does not guarantee admission.
Placement decisions are based on assessment, documentation review, and regulatory criteria.
Who May Submit a Referral
Referrals may be submitted by:

County agencies

Courts
-
Hospitals
-
Behavioral health providers
-
Managed care organizations
-
Community agencies
-
Parents or legal guardians (for adolescents)
All referrals must include sufficient documentation to support review.
Required Documentation
Referral packets may include:

Recent clinical assessment

Diagnostic information (if applicable)

Current treatment plan

Medication list
-
Behavioral history
-
Legal or custody documentation (if applicable)

Insurance or payer information
Additional documentation may be requested as part of the review process.

Admission is contingent upon regulatory criteria, program capacity, and service authorization when required.
Level of Care Determination
AFLH evaluates referrals to determine whether the individual meets criteria for:​

Day Treatment

Case Management

If services are determined to be outside of AFLH’s regulatory scope, referral sources may be provided with alternative resource guidance when appropriate.
Service Authorization
Certain services may require prior authorization from managed care organizations or payers.
Admission and continuation of services are subject to authorization requirements and regulatory standards.

Important Notice
AFLH does not provide emergency services or inpatient psychiatric hospitalization.
If an individual is experiencing a behavioral health emergency, contact 911 or the appropriate crisis response service.
Health and background info is used to plan non-medical support (medication reminders, dietary needs, daily routines).
We do NOT provide direct clinical care.
