Program Overview
AFLH provides case management services through its certified behavioral health agency in accordance with Ohio Department of Behavioral Health regulations and applicable Ohio Administrative Code requirements.
Case management services are designed to support coordination of care, service linkage, and monitoring of individualized treatment plans.
Services are delivered based on documented need and, when applicable, medical necessity criteria.

Clinical Case Management
Clinical case management may include:
