PROGRAM OVERVIEW

A Stable and Structured Residential Environment

AFLH provides structured residential support services for adults who require a safe and supervised living environment.

Our adult residential homes offer consistent support, medication oversight, and coordination with behavioral health providers to promote stability and independence.

The program is designed for adults who benefit from structured residential living while working toward personal growth and community integration

POPULATION SERVED

Our adult residential support services may be appropriate for individuals who:

• Have diagnosed mental health conditions
• Require assistance with medication management
• Benefit from structured routines
• Need supervision to maintain stability
• Are transitioning from higher levels of care
• Require coordination with outpatient providers

All referrals are reviewed to ensure appropriate program fit.

RESIDENTIAL SUPPORT SERVICES

• Supervised residential living
• Medication monitoring support
• Assistance with daily routines
• Case management coordination
• Community resource linkage
• Life skills support
• Transportation coordination as applicable

Our adult homes promote accountability, safety, and personal responsibility within a supportive environment.

STRUCTURE & EXPECTATIONS

Residents participate in consistent daily routines designed to promote stability and self sufficiency.

Program expectations include:

• Respect for shared living environment
• Participation in scheduled activities
• Compliance with house guidelines
• Engagement in treatment services as applicable

Clear structure and expectations support long term success.

CLINICAL & PROVIDER COORDINATION

AFLH adult residential services coordinate with outpatient providers, prescribers, and case managers to ensure continuity of care.

While adult residential services focus on supportive living, we collaborate with licensed clinicians to support residents’ behavioral health treatment plans.

Our team maintains communication with appropriate providers and stakeholders to ensure stability and safety.

SAFETY & OVERSIGHT

Safety is a priority within our adult residential homes.

AFLH maintains written emergency procedures and supervision protocols to ensure the well being of residents and staff.

Homes are maintained in compliance with Ohio residential care standards and safety guidelines.

ADMISSION & ELIGIBILITY

Admission is based on appropriateness for structured residential support.

Individuals must:

• Be medically stable
• Not require inpatient level of care
• Meet residential eligibility requirements
• Demonstrate ability to function within a shared living environment

All referrals undergo review prior to acceptance.
